What will take place?

As part of your initial intake you can expect to receive a thorough clinical assessment from a licensed professional that is comprehensive, addresses mental health, substance abuse and physical health needs. Once the initial assessment is completed, the staff responsible for coordinating your treatment work with you to develop the plan of care based on your individualized needs and preferences. If it is determined through the assessment that you need to see one of our Day-Mont Psychiatrists for medication; your primary therapist/case manager will make arrangements for you to have the next available initial psychiatric evaluation with one of our Medical Doctors.

    1. At least one of the items below that will help verify your residency.
    DOCUMENT(S) MUST BE CURRENT WITHIN 30 DAYS OF VISIT AND INCLUDE COMPLETE COUNTY ADDRESS SAME AS DECLARED COUNTY RESIDENCE (HOUSE/APT NUMBER, STREET, CITY, STATE AND ZIP CODE).

  • Regional Transit Authority (RTA) Disability Pass

    3. Dependent Verification - Documentation that members included in family size are dependents such as tax returns, school records, shot records, court records and/or birth certificate.
    4. If you have a legal guardian, please bring your legal guardian along with proof of legal guardianship.

Day-Mont Behavioral Health Care, Inc. does not provide supervision for children. If it is necessary for you to bring your child, you must have someone with you available to supervise them while you are in Intake interview room.
