This job reports to the Assertive Community Treatment (ACT) Team Manager.

Relocation is not provided and travel is required consistently.

This is a Full-Time position, working 1st Shift.

Job Skills/Requirements

Primary responsibility for developing and implementing individual treatment plans for clients. Provide direct services including, but not limited to, ongoing assessment of activities of daily living and therapeutic engagement. Accompany clients on appointments, act as an advocate for clients to secure needed services and financial entitlements. Attend ACT team meetings daily in order to facilitate an integrated multidisciplinary team approach to service. Must maintain agency required productivity standards, completing all required documentation in a timely and accurate manner. Qualified applicants must demonstrate excellent verbal and written communication skills and excellent computer skills in order to document services in an electronic clinical record. Applicants must have a two (2) or four (4) year degree in social work, psychology, or other behavioral health related field or documented competency/experience in community mental health. Applicants must also have a valid Ohio driver's license, safe driving record and reliable transportation.
